Horizon Care, LLC is a company based in Minneapolis, Minnesota that provides Independent Assist Living services to residents with developmental, mental, and physical disability. Ideally, our services benefit residents who are disabled, homebound or medically-fragile. We help make their lives easier with the professional care we provide in our wonderful assist living facility.
